The space probe Mars Climate Orbiter

On the 23th september 1999, the placing in orbit of the space probe Mars Climate Orbiter failed!
And yet, all had started well:

10 h 41 (English hour): the placing in orbit starts
11 h 01: the engine of 640 Newtons of push is fired
11 h 06: the radio signal of the probe disappears, as envisaged, because the probe is behind Mars
11 h 27: the technicians and the engineers of NASA retain their breath, but the radio signal, so much awaited, does not come .....

It will never come but how that could this happen?
